There are two options one can take to express his/her interest in the Adult Day Habilitation Program!

What to Expect

The Adult Day Habilitation Program accepts individuals with intellectual and developmental disabilities who are 21 years or older. After having already contacted the Adult Program and met our criteria for enrollment, here is what you can expect from us:

Request for Relevant Documentation

We will ask you to provide an up-to-date Individualized Service Plan (ISP) and, if you have graduated high school within the past year, a recent Individualized Education Program (IEP).  

Take A Tour

If you are interested, a general tour of our ten-acre campus is available. Upon completion of the general tour, if there is interest in our Program, an individualized intake can then be scheduled.

You can schedule an individualized intake with our Program Director.
